Fifth Grade KOK Registration

North Allegheny Camp Kon-O-Kwee Website

 

Each family will need to complete the following 4 items for registration (5 if medication is required at camp):
 
  1. NA Printable Waiver Form - needs returned to school
Your child is coming home with the NA waiver in their backpack today.  Please fill it out and return it to school. The form is also available from the KOK website above. 
 
  1. 2024 Participation Registration Form - completed electronically
This form is also available on the KOK website (linked above).
 
  1. YMCA Waiver - completed electronically
The “Camp Kon-o-Kwee Electronic Waiver Form” is also available on the KOK website. Be sure to select McKnight Elementary.
 
  1. Payment - Day Experience - $65; Overnight Experience - $115
Payment can be made by check (payable to North Allegheny School District) or through Pay Schools Central (link also available on the KOK website)

  1. Medical Form - ONLY required if medication is needed during camp
If your child needs to take medicine while at camp, please fill out the medication form that was also sent home and return it to school. The form is also available on the KOK website. There is also a sunscreen form if needed for your student.
